Care Commons supports eligible Medi-Cal members facing challenges related to health, housing, pregnancy, or everyday stability.

Care Commons may be right for you if you:

  • Are expecting a baby or recently gave birth
  • Need help finding or maintaining stable housing
  • Feel overwhelmed trying to coordinate multiple services
  • Need support understanding benefits, appointments, or available resources
  • Want a consistent point of contact to help make things feel more manageable

We work with adults, young people, families with children or other dependents, and older adults.

Every situation is different. Our role is to reduce barriers, ease stress, and help people access the care and resources that can improve daily life.
