Our Approach
Global Communities is dedicated to ensuring every child receives a robust start in life. Funded by the United States Health Resources and Services Administration (HRSA), Healthy Start Enhanced seeks to provide the perinatal care that all families deserve, but only some have access to. The project is utilizing a client-centric method to advance family wellness and decrease Black infant mortality rates in San Bernardino County. Healthy Start Enhanced provides complimentary prenatal and birthing education, postpartum and breastfeeding assistance, mental health Services, doula support services and concierge birthing assistance. All services are designed to improve perinatal and infant health outcomes, reduce infant mortality and foster health equity.
Community Health Workers, known as care coordinators, provide telehealth-based visits for families from the time they are pregnant until their child is 18 months old. Enrolled participants have access to midwifery care, doula and lactations services and concierge birth and resource navigation services.
Community Consortium
The HSE-SB Community Consortium is a dynamic initiative comprised of dedicated individuals with a passion for reducing Black infant mortality rates in San Bernardino County. Our Consortium collaborates to address social determinants of health impacting birth outcomes. Through monthly meetings and action planning workgroups, we prioritize projects to tackle issues such as access to care, LGBTQ+ family support and environmental factors affecting perinatal health. Our Consortium fosters cross-sector partnerships, facilitates training opportunities and ensures community voices drive decision-making. Join us in creating measurable improvements in perinatal health for all residents of San Bernardino County to ensure our babies thrive for generations to come.

Enrollment: Program Eligibility
- Black/ African American (mother/birthing person & father/partner) residing within the San Bernardino County (pregnant – 18-months postpartum)
- Any race/ethnicity that identifies as low-Income, low-Income eligible for Medi-Cal, Cal-Fresh or 200% of the federal poverty level or less
